This place should absolutley not be allowed to operate, they have no interest in care and will drug your loved ones as they see fit to not have to be bothered with them. Absolute garbage there is no rating low enough based on my observations. My mom has moved into Cold Spring Hills Center for Nursing & Rehabilitation.

Having a lack of empathy, care and professionalism are not the characteristics needed in a leadership team. Blaming a patients current decline on their history without fully reading their chart and realizing your staff caused his decline is despicable. Please do not make the mistake we did, stay for away! If you’re not sure if it’s negligence, still report it.
The dining area is not inviting to go sit in. The place is 20 minutes from my house, and the grounds are big. My husband was at Cold Spring Hills Center for Nursing & Rehabilitation. They were kind, accommodating, and efficient.

My mom was dying and died a few days later in the hospital. All they cared about was keeping her there for more money. Husband has been in sub acute rehab since July supposedly to get physical therapy for an amputation. Physical therapy not given anywhere near consistently or adequately to the point where he now has a contracture in the leg and can't get a prosthesis until they fix it. He was given a brace that they don't put on him and when they do its incorrect, medication wrong EVERY day to the point where there mistakes could have life threatening consequences, wound care not done correctly.

The last time this data was collected they cared for at least 259 men and 433 women in a one year period that received Medicare benefits. This number fluctuates with each new resident.
I have to say that the Physical and Occupational Therapists were excellent. I had two amazing therapistswho were not only good at their jobs but wonderful, caring people. Everything else, except one nurse, was awful.They were terribly understaffed and you could press the call button andwait for hours until someone showed up to help.
My father was supposed to be given wound care but I doubt they even realize he had an amputation. When he needs to get assistance to go to the bath room the nurse or aide come about 2 hours after which led him to trying to go himself and falling. He is now in a hospital and getting antibiotics for the infection. Once again this was supposed to be handled by the doctors at the facility. If you have a love one in need of post traumatic card NOT BRING TO THIS PLACE.

A skilled nursing facility may be a freestanding facility or part of a hospital that has been certified by Medicare to admit patients requiring subacute care and rehabilitation. Cold Spring Hills Center for Nursing & Rehab practices should be investigated. Our loved one went for rehab after a long stay in the hospital.

Disgusting place, doesn't deserve a rating of one. It was dirty, the aides could careless and the food was disgusting. The only reason my mom stayed was because she made some lovely friends with the other residents.
We weren’t very happy with them because he didn’t get good care there. At one time, he fell out of bed and was on the floor. He was able to pull the phone down and call me to tell me he was on the floor. I had to call the nurses station and tell them to go check on him. Then in the lunchroom, they left him sitting in a wheelchair staring at the wall. The only thing that was nice was the outside in the courtyard.
Signed in on Kiosk and questionnaire was very user friendly. Lobby was very clean and decor was beautiful. I would definitely consider bringing my loved ones here.
